These forms are for permanent employees working 20 or more hours per week:

Required Payroll/Tax Forms:

• M-4 Massachusetts State Withholding Tax Form 
• W-4 Federal Withholding Tax Form
• I-9 Employment Eligibility Verification - Must Present Acceptable Forms of ID
• Norfolk County Retirement Enrollment Form - Copy of Birth Certificate Required
• Direct Deposit Form - Must Submit Voided Check with Bank Routing and Account Number
• Social Security Statement
• Employee Information/Emergency Contact
• Employee Checklist - TBD
• Declination of Health Insurance Coverage - Complete Only if NOT Enrolling into Town's Medical Insurance Program

Policies and Other Items:

• Conflict of Interest On-Line (State Ethics) Law - Must Provide Certificate of Completion and Summary Confirmation
• Drug & Alcohol Policy ( DPW Union Employee Only) - TBD
• Emergency Contact Form
